I did some experimenting last night and found that it's not how many times you click "Combine" but simply, how many times you click on the object, even if its already open and you click it, those clicks count against you, and you only have to click it about 3000 times and it poofs.
The way to remedy it is simple, all you do is zone and come back and the object you were using whether it be a Forge, Oven, Pottery Wheel or a brewing barrel will be back. Camping also works, just gotta reload the zone.
Another thing they've done recently is made NPC's a little smarter, when you sell mass quantities of an item to a single NPC, he will pay you top value for the item for a long time, but if you continue selling to him, after a while he slowly but surely starts paying you less and less, I'm not quite sure what triggers them to lower their payback price each notch, but I think the trigger is selling him over X number of items in a single trading session, then clicking off him and reclicking him and trying to sell him that same item again. It's a very slow regression in price, he will pay 0.5% (maybe less, just guessing) less each session. If anyone knows the specifics on how this works I'd appreciate the knowledge.
I know you all are good little angels and none of you exploit merchants for profit, afterall that subject is taboo here, and you will burn in hell if you do it, and I certainly don't condone it!
