operator_new isn't solved in the 10-9-2003 v2, so here's the offset
00582EE4 operator_new
to apply the name in IDA do this:
1. hit G for "go to" code location
2. Copy/paste or type 582ee4, hit enter
3. hit N for "rename"
4. Copy/paste or type operator_new, hit enter
done.

Any chance of making this in another format? I have customized my IDA a bit, and most of the old names that you rely on for renaming are not there. How hard would it be for you to produce a version formatted like this:

instead?
