Choose the "For Windows Desktop" version.
VSE2012 does a full build clean build of MQ2 for me in about 21 seconds btw.

I'm not sure what the point of this is. Are you just trying to save disk space on your build machine after a compile?

If you want to know what to copy around to your various EQ PCs, it's just *.exe, *.dll and *.txt (assuming you have already configured the .ini files on those systems).

Primary mappings set unders options->keys (in game) work but those set as alternate don't. Seems like some sort of offset problem since they're kind of scrambled.

Making it work at all is pretty much the same amount of effort as making everything work.
Actual new feature fixes would probably help for monthly patches on live servers too, but getting basic functionality going for every test patch would be significant.
If you care about MQ2, don't play on test.
